Care Agreement 2026
Before Your Visit
It is ideal to return the paperwork prior to your appointment by fax or mail. If you're unable to, please bring the completed forms with you on the day of your visit.
Please also bring:
- A copy of your insurance card(s)
- Any referral or authorization required by your insurance (if not sent in advance)
- A list of your medications with dosages — or your actual pill bottles
Please arrive 20 minutes prior to your scheduled appointment time.
Confirmation & Cancellation
You will receive a confirmation call two days prior to your appointment. If you are unable to keep your appointment, we request a minimum of 24 hours notice to avoid a cancellation fee.
